CAS is the encryption and entitlement system that controls which subscribers can watch which channels. The headend scrambles the signal; each subscriber's set top box (with a smart card or cardless client) descrambles only the channels they paid for. Without CAS, satellite TV is free-to-air; with CAS, it becomes a subscription business.
CAS pricing has three parts: 1) a one-time integration/license fee, 2) a per-subscriber monthly fee (typically starting from a few US cents, scaling down with volume), 3) sometimes a headend license.
